Tonmya: A New Hope for Fibromyalgia

by Melissa Talwar MSN, RN, NBC-HWC | May 31, 2026

Tonmya, a medication from Tonix Pharmaceuticals, has recently hit the market with high hope to help patients better manage fibromyalgia by improving sleep.

Why is Fibromyalgia so tough to treat?

Before diving into Tonmya, let's quickly recap what fibromyalgia entails. According to the current diagnostic criteria from the American College of Rheumatology, fibromyalgia is diagnosed based on widespread pain in at least four of five body regions, symptoms persisting for at least three months, and specific scores are calculated on the Widespread Pain Index (WPI) and Symptom Severity Scale (SSS). Symptoms include pain, persistent fatigue, poor sleep quality, and brain fog. Unfortunately, there's no cure, and existing treatments like pregabalin, duloxetine, or milnacipran don't work for everyone and can come with their own set of side effects.

This is where Tonmya steps in. Approved by the FDA in 2024, it's the first new therapy for fibromyalgia in over 15 years and represents a non-opioid option designed specifically for the condition's core symptoms.

Introducing Tonmya: The Basics

Tonmya (cyclobenzaprine hydrochloride sublingual tablets, 2.8 mg) is a prescription medication developed by Tonix Pharmaceuticals, a company focused on innovative treatments for central nervous system disorders. Unlike traditional oral pills, Tonmya is taken sublingually, meaning it dissolves under the tongue for faster, direct absorption into the bloodstream. This bypasses the stomach and liver, allowing for quicker onset and potentially fewer gastrointestinal issues.

It's indicated for adults with fibromyalgia to help alleviate widespread pain, improve sleep quality, and reduce fatigue. It’s not approved for treating sleep disorders or fatigue independently, and its safety and efficacy in children haven't been established. Patients typically start with 1 tablet at bedtime for the first 2 weeks, potentially increasing to 2 tablets based on their doctor's recommendation. The routine involves moistening the mouth with water, placing the tablet under the tongue after oral care, and avoiding eating, drinking, or talking until it fully dissolves.

How Does Tonmya Work?

Tonmya's active ingredient, cyclobenzaprine, is a muscle relaxant that's been around for decades, reformulated here into a low-dose, sublingual version. It targets the central nervous system to modulate pain signals and promote better sleep, addressing the root causes of fibromyalgia symptoms rather than just masking them. By improving sleep quality, a key factor in fibromyalgia exacerbation, Tonmya aims to break the cycle of pain and exhaustion. This makes it distinct from other treatments, as it's the first bedtime-specific, non-opioid analgesic for fibromyalgia.

Clinical Evidence: What Do the Studies Say?

Tonmya's approval was based on two Phase 3 clinical trials involving over 1,000 participants. In both studies, patients taking Tonmya showed statistically significant reductions in widespread pain compared to placebo. One trial also demonstrated improvements in sleep quality, fatigue, and daily functioning (like walking or carrying groceries), though results were mixed in the second trial. Overall, benefits were sustained over the 14-week study period, with no new safety concerns emerging.

As a first-in-class treatment, it's now the fourth FDA-approved option for fibromyalgia, joining the ranks but standing out with its unique delivery method.

Potential Side Effects and Considerations

Like any medication, Tonmya isn't without side effects. The most common include temporary mouth numbness, discomfort, abnormal taste, drowsiness, tingling, mouth pain, fatigue, and dry mouth. Tonix said these are usually mild to moderate and resolve quickly. Serious risks, noted as rare, could involve serotonin syndrome, heart issues, or oral mucosa problems, so it's crucial to discuss your medical history with your doctor.

Pricing has been a point of discussion in the community. Without insurance, it can be costly, but patient assistance programs and insurance coverage are expanding as they're newly launched. Always check with your healthcare provider and pharmacy for affordability options.

Availability and Next Steps

Tonmya became commercially available in the US in 2025. It's distributed through pharmacies nationwide, and Tonix has been ramping up awareness among physicians and patients. If you're living with fibromyalgia, talk to your doctor about whether Tonmya could be right for you, especially if other treatments haven't provided relief.

Tonmya offers renewed optimism for those battling fibromyalgia. While it's not a cure-all, its targeted approach and positive trial data suggest it could improve quality of life for many.
