Living with fibromyalgia often means watching for any new option that might help manage the constant challenges of widespread pain, fatigue, and poor sleep. Tonmya (cyclobenzaprine hydrochloride sublingual tablets), approved by the U.S. Food and Drug Administration (FDA) in August 2025, has generated significant interest in patient communities since becoming more widely available. It is the first new medication specifically indicated for fibromyalgia in more than 15 years. This post begins with early real-world patient experiences shared online and then reviews the clinical evidence, safety considerations, and practical guidance for those considering this treatment option.
What Patients Are Reporting
Approximately 9 months after FDA approval, discussions on public forums such as Reddit’s r/Fibromyalgia and Facebook groups, and on Mayo Clinic Connect reflect a range of personal experiences with Tonmya. These accounts are anecdotal and not derived from controlled clinical trials; individual responses vary widely.
Some patients report improvements in sleep quality and subsequent daytime functioning. One individual shared, “My sleep has completely changed. I used to have chronic insomnia... Now I am sleeping within an hour and sleeping until 7 or 8 a.m.!” (reddit.com). Another user, relatively new to medications in this class, described broader benefits: “With Tonmya, I can tell that it really helps with my sleep quality and improves next-day symptoms. Brain fog, fatigue, and mood are better. I wake up thinking, ‘Is this kind of like what normal people feel like?’” (reddit.com). Patients who respond positively often note feeling more refreshed and better able to engage in daily activities.
Challenges are also frequently discussed. Common oral side effects, such as tongue numbness, an unusual taste, or discomfort, lead some individuals to discontinue use. Others report minimal symptom relief or next-day grogginess. Cost and insurance coverage remain significant barriers, with high out-of-pocket expenses for patients lacking comprehensive prescription benefits (GoodRx, 2026). These varied real-world reports underscore that fibromyalgia treatments affect each person differently. What provides substantial relief for one individual may offer only modest or no benefit for another.

Background on Tonmya and Clinical Evidence
Tonmya is a low-dose (2.8 mg titrated to 5.6 mg) sublingual formulation of cyclobenzaprine taken once nightly at bedtime. It is designed to address the disrupted sleep architecture often observed in fibromyalgia, aiming to indirectly reduce pain and fatigue (Tonix Pharmaceuticals, 2025a). FDA approval was supported by two positive Phase 3 randomized, double-blind, placebo-controlled trials (RELIEF and RESILIENT). A third trial (RALLY) did not meet its primary endpoint (Tonix Pharmaceuticals, 2025a). The primary outcome in the successful studies was a statistically significant reduction in average daily pain intensity, measured by the Numeric Rating Scale at Week 14, although the magnitude of improvement was modest compared with placebo. Secondary outcomes included improvements in sleep quality, fatigue, and overall function as assessed by the Fibromyalgia Impact Questionnaire-Revised (Tonix Pharmaceuticals, 2025a; Lederman et al., as cited in Tonix Pharmaceuticals, 2025a).
Common adverse reactions in the trials were generally mild and transient, primarily consisting of oral discomfort or numbness. These findings satisfied the FDA’s long-standing requirement of two adequate and well-controlled studies to establish substantial evidence of efficacy for new indications.
Important Safety Note on Drug Interactions and Serotonin Syndrome
Tonmya carries warnings about potential interactions, particularly the risk of serotonin syndrome, a potentially life-threatening condition. According to Davis's Drug Guide, this risk increases when combined with other serotonergic medications, such as SSRIs, SNRIs, tricyclic antidepressants, tramadol, bupropion, meperidine, verapamil, or MAO inhibitors. Concomitant use with MAO inhibitors is contraindicated. Symptoms of serotonin syndrome can include mental status changes (confusion, agitation, hallucinations), autonomic instability (rapid heart rate, high blood pressure, fever), neuromuscular issues (tremor, rigidity), and gastrointestinal effects (nausea, diarrhea). Patients should inform their provider of all medications and supplements before starting Tonmya. If symptoms appear, treatment should be stopped immediately, and medical help should be sought (Vallerand & Sanoski, 2025).
Practical Thoughts for Patients
Patients considering Tonmya should discuss its potential role within the broader fibromyalgia management plan with their healthcare provider. Some clinicians may consider lower-cost generic formulations of cyclobenzaprine for appropriate candidates. Maintaining a simple symptom diary tracking sleep, pain levels, and daily function over the first 4–8 weeks can help determine individual response. As with any pharmacologic therapy, Tonmya is most effective when combined with evidence-based nonpharmacologic strategies such as nutrition, consistent sleep hygiene, stress management, and gentle movement.
A thorough review of medical history, including any prior reactions to muscle relaxants, is essential prior to initiation.
What Has Your Experience Been?
